Yes. I used a template because I didn't know how to get lines under the titles of each section but I deleted everything else and made it all black and white. Go to just about any university's website and they probably have some out lines. Down below is a link to the career and resume book where I graduated from. Page 16 is where the resumes start.

No it is not unprofessional. Not using a template when you don't know what you are doing would be unprofessional.
No problems, just leave off any fancy graphics as most companies use software to ';read'; and the graphics just mess them up.
